Hospital Transfer Form for California Licensed Midwives
LMPA dictates that hospitals submit a transfer form for all licensed midwife transfers. Although, the responsibility of this is the hospital's, licensed midwives can assure more accurate information by pre-filling out the form. The midwife can include information such as midwife name, LM#, reason for transfer, whether report was given to the physician and whether prenatal records were provided. The form is available on the medical board website here.
Homebirth Summit Guidelines
These guidelines were developed by a multidisciplinary group
of home and hospital based providers and stakeholders who were delegates at the
national Home Birth Consensus Summits in 2011 and 2013. These guidelines are
informed by the best available evidence on risk reduction and quality
improvement and by existing regional policy and practice documents addressing
transfer from home to hospital. Read
